All tournament paperwork for team check-in will be done online via GotSoccer.
Please note your documents must be verified once uploaded. 

Please have all documents uploaded by 4/19/19.

USYSA Requirements (IYSA, MYSA, WYSA, etc.)

Teams from Illinois (IYSA)

  • Illinois Tournament Roster (NOT GOTSOCCER ROSTER)
  • State Roster
  • Guest Player Forms (if needed)
  • Player Cards (front & back so includes picture of player)
  • Risk Management Form

Out of State Teams (MYSA, WYSA, etc.)

  • Illinois Tournament Roster (NOT GOTSOCCER ROSTER)
  • State Roster
  • Guest Player Forms (if needed)
  • Player Cards (front & back so includes picture of player)
  • Risk Management Form
  • IYSA Medical Release Forms
  • Travel Permit

US Club Requirements
